Nurse Practitioners must have 2+ years experience in geriatrics, emergency medicine, or multi specialty medicine.

We are seeking Nurse Practitioners to join our growing Multi location practice that concentrates on treating patients for primary care in Assisted Living facilities and an ambulatory Office. As a Visiting Physicians Health team provider, you will focus on providing high quality patient care.

Responsibilities:

• Diagnose and treat acute, episodic or chronic illnesses

• Prescribe necessary medications and treatments

• Perform comprehensive physical examinations of patients

• Develop and implement patient management policies and procedures

• Facilitate referrals to other healthcare professionals and medical facilities

• Communicate with collaborating physician and patient's family regarding patient care

Qualifications:

• Nurse Practitioner 2+ years experience

• Deep commitment to providing complete care

• Previous experience in Assisted living, long term care and or ambulatory setting

• Ability to build rapport with patients and family members

• Ability to thrive in a fast-paced environment

• Excellent written and verbal communication skills

• EHR experience

Job Type: Full-time/Part-time

10-40 Hours per week
