Description:

The antimicrobial stewardship pharmacist is responsible for overseeing and directing the use of antimicrobials to ensure their appropriate utilization and to optimize patient outcomes in the treatment of infectious diseases. In collaboration with infectious diseases physicians, infection preventionists, and hospital leadership, this position will facilitate the development and implementation of evidence-based processes that promote antimicrobial therapy that is efficacious, safe, and cost-effective.

Job functions:

• Conduct daily antimicrobial stewardship activities including prospective audit and feedback and review of restricted antimicrobials

• Participate in multidisciplinary antimicrobial stewardship rounds with infectious diseases physicians

• Design, implement, and monitor performance improvement initiatives in collaboration with pharmacy, infectious diseases, clinical microbiology, infection prevention, nursing, and quality

• Provide antimicrobial stewardship performance updates to key stakeholders through both institutional and regional committees

• Share expertise on antimicrobial selection and dose optimization with pharmacy and non-pharmacy staff

In accordance with the Mission and Guiding Behaviors the Patient Care Pharmacist, within the scope of the Ohio Pharmacist Practice Act, will provide timely, accurate and cost effective pharmaceutical care.

  • Ensures an environment of patient safety, promotes evidence-based practice and quality initiatives and exhibits professionalism

Requirements/Qualifications

  • Bachelor or Doctor of Pharmacy required with two years of institutional pharmacy experience with specialty training in infectious diseases preferred or completion of a PGY- 2 Infectious Diseases Pharmacy Residency.

  • Current license to practice as a Registered Pharmacist in the State of Ohio
