Scheduled Weekly Hours:40 Gundersen-Tri County Hospital and Clinics is seeking a full-time Physical Therapist to join the team working primarily at our new clinic location in Mondovi, WI. This is your opportunity to be a part of a growing health system focusing on staff, patients, and the communities we serve! What you will do: Work full-time (40 hours/week) Monday-Friday day shifts Monday, Wednesday, and Friday in Mondovi, Tuesday & Thursday in Whitehall to start. As the practice grows in Mondovi, the work will transition to full-time at the Mondovi location. Job Description: The Physical Therapist in their clinical role examines, evaluates and assesses individuals of all ages who have medical problems or other health related conditions that limit their ability to move or function with activities of daily living. The Physical Therapist develops a plan of care and provides treatment techniques and interventions to improve mobility, reduce pain, restore function and prevent disability. The Physical Therapist provides physical therapy services and elements of patient/client care management leading to optimal outcomes according to the relevant state Physical Therapy Practice Standards. The Physical Therapist works collaboratively with a multidisciplinary team in a team in the areas of education and consultation regarding patient needs in accordance with the vision and mission of Gundersen Health System. Major Responsibilities: 1. Works with patients and families to examine, evaluate, diagnose, prognose, and intervene to insure optimal outcomes. Also consults and educates the patient and other care-givers as needed. 2.
