Where do you fit?

To be a successful Independent Living Coordinator you must become able to do the following always:

  1. Ensure a trauma informed focus on creating a safe and supportive environment for staff and those served through knowledge, sensitivity, respect, and a focus on resilience and recovery.
  2. Teach relevant life skills. Demonstrate the ability to transform daily living activities into learning opportunities to assist youth in the development of independent living skills.
  3. Provide services to improve youth proficiencies in independent living skills by:
    1. Maintaining a maximum caseload of 20 total young adults, including 8 Scholarship Phase youth.
    2. Providing direct consultation, direct service, and referrals for all department youth.
    3. Developing an individualized transition plan in consultation with youth reflecting their needs. Facilitate transition plan meetings separate from regular case plan meetings.
    4. Visiting with youth face-to-face in their home or community as identified in the learning plan.
    5. Helping youth access local resources in their community to progress in academics and/or the workforce.
  4. Direct service to Scholarship Phase youth, including Learning Plan development and completion of the Casey Life Skills Assessment.
  5. Direct service to Action Phase youth, including learning plan and Casey Life Skills Assessment provision.
  6. Support Department of Children and Families (DCF) efforts to implement SOUL [Support, Opportunity, Unity, and Legal Relationships)
  7. Documentation, training, transportation, and liaison with DCF IL Coordinators.
  8. Follow Company Policy and Procedure and operate according to the Companys Program Practice Guidelines.
  9. Back up on-call to the on-call department.

Requirements

  1. Bachelors degree in human services or a related field preferred,
  2. Work Experience One child welfare and/or mental health serving adolescents or young adults preference given to work experience with independent living resources and curriculums.
  3. 21 years of age. Must pass a background check, fingerprinting, and physical and drug screening.
  4. Maintain company-required training.
  5. Physical working requirements - Ability to travel, drive Company or personal vehicle including at night. Ability to do occasional lifting of up to 50 lbs. Able to walk stairs. Ability to work flexible schedule, including some evenings and weekends. Employees are subject to outside environmental conditions occasionally, inside conditions frequently, with protection from weather conditions but not necessarily from temperature change.
