What Were Looking For

The Intellectual/Developmental Disability (I/DD) Care Manager is responsible for providing complex care coordination and monitoring to members having a primary Intellectual or Developmental Disability, which may include a secondary mental health or substance abuse, and/or complex medical/physical health need. This position requires a dynamic, proactive approach to assessment, monitoring and complex coordination of care, to ensure quality support and consistent adherence to waiver requirements. IDD/Care Manager assists with integrated care management through multi-disciplinary collaboration.

On a typical day, you might:

  • Utilize person centered planning methods and strategies to gather information, to get to know the members supported and to adequately address specific needs of the individual.
  • Provide education and support to members on learning about and exercising rights and responsibilities.
  • Complete discovery activities (information gathering and assessment) as a component of the planning process.
  • Ensure that assessments and plans are updated as needed and whenever the members life circumstances change.

Qualifications: Education: Bachelors Degree OR Masters Degree OR Fully licensed by the NC governing board regulating Human Services professions Experience: Must meet QP status.
