Wage - \$15.00 per visit for Diabetic Check-Ins - Check-ins usually take no more than 15 minutes per visit Daily Diabetic Schedule (for reference only) 8:30 am, 12:00 pm, 5:00 pm, and 7:30 pm (not to exceed one hour) Tasks Include * Minor assistance with diabetic checks * Diabetic charting Ideal Candidate Traits - Knowledgeable about diabetes, insulin, and blood sugar management or willing to learn - Reliable, friendly, positive, and willing to go the extra mile. - Family-oriented and compassionate - Able to follow instructions and provide the assistance the member prefers - Around 1 year of experience in mental health. - Has a dependable vehicle - Favorable background check Community Horizons proudly supports individuals enrolling in self-directed programs through Managed Care Organizations or IRIS (Include, Respect, I Self-Direct) throughout Wisconsin. Self-direction allows participants the freedom to choose who provides their services and how they receive them.
