Wage: \$15.00 per hour Hours: Let us know what works for you Part-Time to Full Time Weekends Needed Shift Coverage: 7 am - 3 pm, 3 pm - 10 pm, and overnight 10 pm -7 am Potential for pick-up shift during the week Back-to-back shifts are allowed so you can get several hours in 1 -2 days. Tasks Include but are not limited to: All Personal care Changings Transfers with a Hoyer lift Feeding Cooking / Cleaning / Laundry Medication Administration Ideal Candidate: Has 1 year of professional experience in a healthcare-related field Is familiar with the needs of someone with Multiple Sclerosis (MS) Someone who has personal care experience including the use of a hoyer lift Someone who can make conversation about pretty much anything COVID-19 vaccinated preferred or willing to wear a mask when in close contact Has a reliable form of transportation Favorable Caregiver Background check Community Horizons offers support for individuals enrolled in self-directed programs through Managed Care Organization or IRIS (Include, Respect, I Self-Direct) throughout Wisconsin. Self-direction gives program participants the choice over who provides services and how they receive them. Community Horizons is not the employer on record, members make all hiring decisions with the involvement of a third-party fiscal agent responsible for payroll. Maintain records of patient care, condition, progress, or problems to report and discuss observations with supervisor or case manager. Provide patients with help moving in and out of beds, baths, wheelchairs, or automobiles and with dressing and grooming. Care for patients by changing bed linens, washing and ironing laundry, cleaning, or assisting with their personal care. Entertain, converse with, or read aloud to patients to keep them mentally healthy and alert.
