General Summary
Level I Pharmacists provide prescribed pharmaceuticals and drug information as needed for patient care according to professional standards and practices. The Level I Pharmacist role includes routine distributive and clinical services with time in those domains contingent upon the needs of the facility and department. Level 1 Pharmacists may advance to a Level II Pharmacist position based upon job performance and successful completion of the requirements for advancement established in the Pharmacist Career Ladder Program (PCLP) criteria grids.
Duties and Responsibilities
Essential Functions:
Maintains the requirements located in the PCLP Mandatory and Optional Criteria Grids for the Level I Pharmacist.
Provides oversight of safe, accurate, and timely medication order verification including clarification of any medication orders with discrepancies prior to dispensing.
Provides pharmacy management and oversight of: - Integrity and security of medications in all care locations - Investigational drugs - Preparation of injectable chemotherapy - Medication wastage, returns, and recalls - Reconciliation of controlled substance wastage - Compounding of formulations - Automated dispensing cabinets - Medication label components and label quality
Provides medication therapy management services in collaboration with other members of the health care team including consultative services pursuant to the facility's available protocols which may include: - Pharmacokinetic evaluation, monitoring, and dosing (initiation and daily follow-up) - Total parenteral nutrition (TPN) consults - Anticoagulation initiation and follow-up pursuant to a consult - Renal dose adjustments - Intravenous to oral conversions
Provides educational services to patients and healthcare providers regarding medication therapy and/or medication safety concerns.
Provides medication event prevention, monitoring, and reporting with an increased emphasis on patient safety.
Utilizes technology to enhance patient safety by ensuring the proper distribution, monitoring, and use of medications.
Adheres to WellSpan Health's medication-use policies and standard work.
Qualifications
Minimum Education:
- Bachelors Degree Pharmacy Bachelor's Degree Required
Work Experience:
- Less than 1 year Hospital experience. Preferred
Licenses:
- Licensed Registered Pharmacist Upon Hire Required
Courses and Training:
- Completion of 5 year program from accredited school of pharmacy. Upon Hire Required
